Introduction
The United States’ health care delivery system is facing a rising challenge: care for the increasing number of chronically ill. In 2000, almost 125 million Americans had at least one chronic condition, with 60 million people having multiple conditions. By 2020, almost 157 million people will have a chronic condition, with 81 million Americans having multiple chronic conditions (Partnership for Solutions 2002). The aging of the baby boomers (76 million people born between 1946 and 1964) fuels this growth in the number of chronically ill. With annual premium hikes already in the double-digits, boomers are expected to add new fuel to rising costs as they seek out advanced care for rapidly growing cases of multiple chronic illnesses such as cardiovascular disease, depression, and diabetes (Carroll 2003).
